Lafayette - Service Providers
 
Service:

County Connection Link provides service in Central Contra Costa County in the cities and towns of Alamo, Clayton, Concord, Danville, Lafayette, Martinez, Moraga, Orinda, Pleasant Hill, San Ramon and Walnut Creek.


Service Hours:

Paratransit service is provided during the same hours as the regular fixed route service. Additional hours are provided on behalf of BART.


Eligibility:

In order to be eligible for County Connection LINK service you must reside in Central Contra Costa County. It must be determined that you are eligible for paratransit services under the Americans with Disabilities Act (ADA). A person can be found to be ADA paratransit eligible for all or for some of their transit trips, based on the individual’s specific condition(s).


Application Required for County Connection Link:

To apply for LINK eligibility, you may come to the LINK office at 2477 Arnold Industrial Way in Concord, download an application, or call (925) 680-2138 to have an application sent to you. Fill out the application as completely as you can, including whether or not you travel with an attendant or a service animal (i.e. guide dog). It’s OK to have someone help you.

After your application is received and reviewed by a health care professional:

  • You may receive a telephone call for additional information.
  • Your application may be sent to the professional you have indicated on your application.
  • And/or: You may be asked to come to the County Connection offices for an in-person interview.

You will be notified by mail of your eligibility status within 21 days of receipt of your completed application.

The guiding principle for ADA paratransit eligibility is the inability to independently use the fixed-route transit due to a disability or a health related condition.

Service:

Mobility Matters operates two volunteer driver programs that provide free, one on one, door through door, escorted rides by screened and trained volunteer drivers using their own cars. Ambulatory members may bring/use their own equipment. Members who use a wheelchair may be accommodated with our wheelchair accessible van


Eligibility--Rides for Seniors:

  • Live anywhere in Contra Costa County
  • Age 60 or older
  • Require escorted, door through door assistance
  • Able to call in to request rides
  • Unable to use other forms of transportation for medical or dental care and basic necessities, like grocery shopping
  • Trained, certified service dogs welcome


Eligibility--Rides 4 Veterans:

  • Honorably discharged veterans of any age
  • Live anywhere in Contra Costa County
  • Require escorted, door through door assistance
  • Able to call in to request rides
  • Unable to use other forms of transportation for medical or dental care and basic necessities, like grocery shopping
  • Trained, certified service dogs welcome


Application:

Telephone interview with a follow up in home assessment. The application to be completed by a staff member.

Rides For Seniors / Mobility Matters
 
Eligibility:

Residents of Lafayette, Moraga and Orinda—Age 60 and up.


Service Area:

Destinations within Lamorinda as well as to Concord, Martinez, Pleasant Hill, and Walnut Creek.


Hours of Operation:

Weekdays: 9:00 am - 5:00 pm./P>

Application:

Registration form should be mailed to Lamorinda Spirit Van Program, Lafayette Community Center, 500 St. Mary’s Road, Lafayette, CA 94549.


Fees:

Subject to Change. Rides to errands and appointments are currently $5.00 one-way, $10.00 round-trip for destinations within Lamorinda and $10.00 one-way, $20.00 round-trip for destinations outside of Lamorinda. Rides for low-income residents are $5.00 one-way and $10.00 round-trip to destinations within Lamorinda as well as to Concord, Martinez, Pleasant Hill, and Walnut Creek. Rides to the Café Costa are $2.50 one-way, $5.00 round-trip. Call when you make your appointment or at least two business days ahead of time by 1:00 p.m. to schedule your ride. We love to say, “Yes!” so advance notice is helpful. Rides are available to: errands, medical/personal appointments, shopping, workshops, social visits, and the Walnut Creek Café Costa.

Lamorinda Spirit Van
 
Eligibility:

Residents of Lafayette only—Age 70 and up.


Service Area:

Any area serviced by Uber or Lyft.


Hours of Operation:

24 hours a day, 7 days a week.


Application:

To register call (925) 283-3534, weekdays 1:00 pm - 4:30 pm.


Fees:

Subject to Change. Registered clients must call GoGoGrandparent at 1-855-464 6872 to schedule a ride with Uber or Lyft and access the currently available ride subsidy of 50% of the cost of rides up to $50 a month.
